Health Services Case Manager - Full Time - Emergency Shelter

Schedule: Monday-Friday 8:00am-4:30pm.

Rate of pay: $23.00-$26.00 per hour.

The Rescue Mission is actively seeking a full-time Health Services Case Manager to join our dynamic team. This position plays a vital role in supporting the medical and behavioral health needs of residents at our Syracuse Adult Homeless Shelter .

This position is key to coordinating care, promoting access to healthcare, and enhancing the continuum of services that help our clients heal, recover, and thrive.

Key Responsibilities:

• Coordinate and schedule on-site and off-site medical, mental health, and substance use appointments .
• Assist clients with medical paperwork and benefits applications , ensuring timely completion.
• Track attendance , follow-up care , and client health outcomes , maintaining detailed, confidential records .
• Accompany or arrange transportation for clients to external healthcare visits when needed.
• Act as a liaison between clients, shelter case managers, and healthcare providers , supporting care coordination and communication.
• Collaborate with internal teams to integrate health needs into client case plans.
• Follow up with providers for documentation, treatment updates, or referrals.
• Participate in care coordination meetings and ongoing staff development .
• Foster supportive, respectful relationships with clients while maintaining professional boundaries and confidentiality.
• Support outcome reporting and engagement data to enhance program effectiveness.
• Refer clients to appropriate services and respond to community partner outreach within three business days.
• Maintain a safe, structured environment aligned with the Rescue Mission’s mission, vision, and values .

Requirements:

• Education: Associate’s degree required ; Bachelor’s degree preferred in Social Work, Public Health, Human Services, or a related field.
• Previous work experience with homeless or underserved populations strongly preferred.
• Knowledge of local healthcare and behavioral health resources is essential.
• Strong organizational, communication, and interpersonal skills .
• Ability to maintain boundaries , manage sensitive information, and work in a fast-paced, high-demand environment.
• Familiarity with trauma-informed care and harm reduction approaches is a plus.
• Computer proficiency and accurate documentation skills required.
